C CCC标志外环长直径 C CCC标志颜色 Long diameter of CCC Color of CCC mark 毫米mm Mark outer ring 整体 设计 领导形象设计圆作业设计ao工艺污水处理厂设计附属工程施工组织设计清扫机器人结构设计 图Integrated design drawing on affixing position 设计图对应的CCC证书编号 Applicable for the following CCC Certificate(s) CCC非标准规格标志印刷模压申请表填写指南 1.申请人:公司名称,应按照CCC证书中委托人，或申请人，的名称填写。 2.制作方式:申请使用CCC标志的制作方式。 3.标志加施位置:CCC非标准规格标志的加施位置,例如本体,铭牌等。 4.生产厂:生产厂家名称,应按照CCC证书中生产企业名称填写。 5.工厂代码:由认证机构发放,一般在CCC证书生产厂后标注。若未标注,应咨询认证机构。 6.产品名称及型号:应按照CCC证书中产品名称及型号填写。 7.邮寄地址及邮编:接收批准书的地址和邮编。 8.CCC标志单独尺寸图:应包括实际使用时的颜色及CCC标志外环长直径。除特殊式样外,CCC非标准规格标志应与CCC标准规格标志成比例缩放。 9.整体设计图:应含有CCC标志,通常还包含产品型号等。标志加施位置可以是铭牌、产品本体、包装。 10.设计图对应的CCC证书编号:整体设计图所适用的《中国强制性产品认证证书》编号。 提示: 1.如代理机构进行申购，非证书申请人或生产厂，,还需提供CCC证书申请人或生产厂加盖公章的委托书原件,须有授权人签字。 2.以下产品需要在CCC标志单独尺寸图中标注工厂代码:机动车辆及安全附件、电线电缆、机动车辆轮胎、安全玱璃、装饰装修材料、玩具、消防产品、安全技术防范产品。 3.若整体设计图在表中无法完整体现,应单独提供设计图。 4.相同申请人、生产厂及工厂代码、标志加施方式、加施位置可填一份申请表。 How to fill 1. Applicant: Company Name, please keep in accordance with CCC Certificate. 2. The way of Mark processing: In which ways the Irregular-size CCC Mark be added (such as: Pressed, molded, and screen-printed) 3. Mark position: Please indicate where to place the mark 4. Factory: Factory name should be kept in accordance with CCC Certificate. 5. Factory code: Issued by certification body, please keep in accordance with CCC Certificate. 6. Product name and model: The name and model of product showed on the CCC certificate. 7. Postal address and postal code: Address and postal code of receiving Irregular-size CCC Mark Printing/Pressing Permission. 8. Figure for size specification of Printing/Pressing Irregular-size CCC Mark: The size and color of Irregular-size CCC Mark should be indicated in use. The Irregular-size CCC mark must be in proportional line to the Standard-size and should not be distortion, unless the exception approved). 9. Integrated design drawing on affixing position: Integrated design drawing should contain Irregular-size CCC Mark, normally include in product models and etc. Position can be in the nameplate, product body or packing. 10. Applicable for following Certificate(s): Corresponding CCC Certificate(s) number for above mentioned whole design. Note: 1. The authorization letter with signature and enterprise seal by the applicant or factory in case of the application made via an entrusted agent. 2. Additional factory code should be affixed into the figure for following products: safety parts of motor vehicles and, electrical wires & cables, motor vehicle tyres, safety glasses, coating, toys, fire fighting equipment, detectors for intruder alarm systems. 3. The integrated design drawing could be attached if necessary. 4. If the products with same applicant, factory & factory code, Mark position, the way of Mark processing, it can be fill-in one application form.
