BEDFORDHOSPITALNHSTRUSTASTRATEGYFOR1999-CONTENTS1.PurposeoftheDocument2.ThePhilosophyoftheHospital3.TheNewNHSEnvironment4.ObjectivesoftheHospital5.SpecificObjectives1.PurposeofthisDocumentThisdocumentoutlinesthestrategicdirectionforBedfordHospitalNHSTrust(‘TheHospital’)forthenextfiveyears.ThisstrategydemonstrateshowBedfordHospitalseesitselfdevelopingoverthenextfiveyearsinlinewiththehealthcareneedsofourlocalpopulationandtheexpressedstrategicintentionsofthelocalHealthAuthorityPrimaryCareGroupsandotherpartnerorganisationsasdescribedintheBedfordshireHealthImprovementProgramme.Aswithanystrategythisdocumentdescribesadirectioninwhichweintendtotravel.Howeverthisdocumentwillbereviewedandamendedasfactorswhichinfluencethehospitalalterandcreatedemandsforeitherdifferentservicesorafocusondifferentpriorities.ThisdocumentisnotintendedtobeexhaustiveanddoesnotincludedetailonstrategicdirectionordevelopmentsrelatingtospecificserviceareaswhichwillformpartoftheTrustsServicePlanningcycle.ThedocumentbeginsbydescribingthephilosophyofBedfordHospitalfollowedbythecontextinwhichthisstrategyhasbeendevelopedandthenthekeystrategicobjectivesoftheHospitalwithdetailsoftheachievementswhichtheHospitalintendstomake.2.ThePhilosophyoftheHospitalThephilosophyofBedfordHospitalistoprovideexcellentserviceswithwelltrainedandmotivatedstaffrespondingtothehealthcareneedsofthecommunityofBedfordshireandsurroundingareaswithinthefinancialresourcesavailable.BedfordHospitalwillprovideservicesbasedupontheNHSprincipleoffreeandaccessiblehealthcarewhenandwherepeopleneedit.ThehospitalwillstrivetoberegardedasthebestDistrictGeneralHospitalintheRegion.ThehospitalwillcontinuetoprovidecoreservicesappropriatetoaDistrictGeneralHospitalwhichhasanA&EdepartmentwhilstalsodevelopingawiderangeofspecialistservicesinpartnershipwithotherNHSbodies.ThehospitalwillcontinuetopromotetrainingandresearchincollaborationandpartnershipwithotherNHSandnon-NHSorganisations.3.TheNewNHSEnvironmentTheenvironmentwithinwhichtheHospitalisoperatingischangingradically.GovernmentpolicyhasshiftedtheemphasisoftheNHSawayfroman‘internalmarket’baseduponcompetitionandtowardsa‘wholesystemsapproach.’Thisapproachisbasedoncollaborationandco-operationbetweenNHSorganisationsandinpartnershipwithotheragencies.ThischangingenvironmentnecessitatesachangeinNHScultureandthewayinwhichtheplanningandprovisionofNHShealthcareisundertaken.InordertosupportthisnewagendatheHospitalwillworkcloselywithotherorganisationswithinBedfordshireandbeyondinordertoensurethattheserviceswhichareplannedanddevelopedbytheHospitalcontinuetomeettheneedsofthepeopleweserve.4.ObjectivesoftheHospitalOperatingwithinthischangingenvironmentandwithintheframeworkoftheBedfordshireHealthImprovementProgrammetheHospitalhasidentifiedfivekeyobjectivesfortheperiod1999-.Withintheseoverarchingobjectivesareidentifiedaseriesofspecificobjectiveswhichareoutlinedinsection5below.Duringthenext5yearstheHospitalobjectivesareto:ContinuetodevelopBedfordHospitaltoprovidethehighestqualityofcaretothepeoplewhousethehospitalanditsservices.WorkactivelyinpartnershipwithotherNHSandnon-NHSorganisationsandthelocalcommunitytoreducetheinequalitiesinhealthandachieveequalityofaccesstohealthcareforlocalpeople.Achievecontinuousimprovementinemploymentpracticesandbeagoodemployerrecruitingandmaintainingawelltrainedandmotivatedworkforce.Provideanenvironmentwithinandaroundthehospitalwhichispleasantaccessiblesafewellmaintainedcosteffectiveandsustainable.MaintainstrongcorporategovernancewithintheHospitalandensurefinancialsecuritywithinthecontextofserviceagreementssecuredwithPrimaryCareGroups.5.SpecificObjectives.5.1ContinuetodevelopBedfordHospitaltoprovidethehighestqualityofcaretothepeoplewhousethehospitalanditsservice.5.1.1TheoverridingobjectivefortheHospitalistoensuretheprovisionofhighqualitypatientfocussedcareforthepeoplewhouseourservices.TheHospitalwillensurethatthequalityofalltheservicesitprovidesarecontinuouslyreviewedandwhereverpossibleimproved.Thiswillbedonethroughon-goingclinicalauditandtheestablishmentofarobustclinicalgovernancemechanism.5.1.2AspartofthedesiretocontinuallyreviewservicestheHospitalPhaseIIIdevelopmentplanfortheSouthWingsitewillreviewtheprovisionofServicesforOlderPeopleandgeneralmedicalservices.Thiswillaimtofurtherdevelopthewayinwhichtheseservicesareprovidedandimprovetheaccommodationinwhichtheyarehoused.ThePhaseIIIdevelopmentplanwillalsoallowareviewoftheprovisionofotherareasofpatientcaresuchasphlebotomyandoutpatientservices.5.1.3TheHospitalwillseektofurtherdeveloprelationshipsandlinkswithotherlocalproviderssuchastheLutonandDunstableHospital.Thiswillensurethedeliveryofhighqualityclinicalcaretoapopulationbaseappropriateforspecialistservices.Increasinglyanationalfocusonservicequalityisstressingtheneedforhospitalservicestoservealargeenoughpopulationtoenablecliniciansprovidingspecialistcaretomaintainhighlevelsofskillsandexperienceinarangeoflesscommonconditions.Thismeansthatinfuturesomespecialistservicesmaybeconcentratedatcertainhospitals(possiblyaspartofa‘hubandspokeapproach’toserviceprovisionacrossmorethanonehospital)wherethiswilldeliverincreasedbenefitstopatients.TheHospitalwillachievethisobjectiveandprovideservicesonthebasisofresearchevidencebestpracticeandnationalstandardsbydevelopingnetworksofcarewithotherproviderswhichdelivergreaterbenefitsforpatients.Thiswillmaintaintherequiredskillsandtraininglevelsforstafftoprovidespecialistserviceswithinthehospitalandacrossthecounty.TheHospitalwillseektoleadtheprovisionofsomeserviceswhilstactingasa‘spoke’locationforambulatoryanddaycasecareforothers.5.1.4TheHospitalwillseektodevelopfurtherrelationshipsandlinkswithmoredistantprovidersinordertoensurepromptaccessforlocalpeopletohighqualityspecialisttertiarycarewhichisnotavailablelocally.TheHospitalalreadyworkscloselywithhospitalssuchasAddenbrookesandPapworth.WiththecreationofanewEasternRegionoftheNHSExecutivetheHospitalwillincreasinglylooktosuchlinkagestoprovidespecialisttertiarycareforpatients.Examplesoftertiarycareareasareneurosurgerycardiacsurgeryandradiotherapy.5.1.5LocallytheHospitalwillseektosupportprimaryandcommunitycarepractitionersintheprovisionofcareforpatientsinthecommunity.Thiswillbecarriedoutinpartnershipwithsocialservicesandotheragenciesinparticularsupportingthedevelopmentofintermediatecareservices.BydoingthistheHospitalaimstopreventunnecessaryadmissionstoacutehospitalbasedbedsandensurequickandappropriatedischargetherebyensuringthatscarceresourcesareusedforthesickestpatients.TheHospitalwillseektoachievethisbydevelopingclosecollaborativeworkingwithlocalPrimaryCareGroupsandothers.Thiswillenableclinicaldiscussionsaboutspecificissuesandasharedapproachtocareforpatientsacrossbothprimaryandsecondarycare.5.1.6TheHospitalwillcontinuetodevelopitscommitmenttothetrainingofallmedicalandnursingstaffandstafffromotherdisciplinesandprofessions.Thiswillincludethefurtherdevelopmentoflinkagesandrotationsforstaffwithotherprovidersandwithinthehospitalitself.TheHospitalwillseekfromthistoachieveadequatestaffinplacetosupportthedevelopmentofspecialistservicesandtoensuretheavailabilityofawelltrainedandhighqualityworkforcetocontinuetoprovidesuchservices.5.1.7TheHospitalwillcontinueitscommitmenttopromotingresearchanddevelopmentacrossallareasofserviceprovisionworkingwithbothNHSandnon-NHSorganisationsasappropriate.5.1.8TheHospitalwillensurethatcorehighqualitylocalservicescontinuetobeavailableandareeasilyaccessibletolocalpeople.ServicessuchasA&Eemergencymedicineemergencysurgerygynaecologymaternitypaediatricorthopaedicandtraumaandserviceswillremainavailableinalocalsettingwhenandwherepeopleneedthemmost.ThecontinuingprovisionofafullA&Eservicealsonecessitatesarangeofotherservicestobeprovidedlocally.TheHospitalwillseektoprovideallservicesinamannerwhichispatientfocussedandwhichofferspatientsandtheircarersprivacydignityandrespect.5.1.9InadditiontothedevelopmentofawiderangeofspecialistservicestheHospitalwillseektocontinuetodevelopotherareasofcaresuchastheprovisionofmoredaycasesurgery.5.1.10TheHospitalwillseektocontinuetoimprovepatientrecordkeepingtoensurethatrecordsystemsareconsistentreliableandcompatible.Theprovisionofaccurateandtimelyclinicalinformationwillbekeytothefutureperformancemanagementandnationalcomparisonsofhospitals.5.2.ToworkactivelywithotherNHSandnon-NHSorganisationsandthelocalcommunitytoreducetheinequalitiesinhealthandachieveequalityofaccesstohealthcareforlocalpeople.5.2.1TheHospitalwillaimtoplayafullpartinthedevelopmentofthelocalHealthImprovementProgramme(HImP)inordertoreducehealthinequalitiesandmaximisethehealthoflocalpeople.TheHospitalwillachievethisbyworkingcloselyandcollaborativelywithallotherorganisationsinvolvedindevelopingtheHImP.Thiswillhelptodeterminethewayinwhichhealthandsocialcareservicescanbestbedeliveredtoachievetheseaims5.2.2TheHospitalwillseektopromotethereductioninhealthinequalitiesofthelocalpopulationandtheimprovementofthehealthofthemostdisadvantagedandvulnerablesectionsofthecommunity.ThiswillbeachievedbyworkingwithotherhealthandsocialcareagenciestodevelopconceptssuchaslocalStrategicPartnershipBoardsandtosupportSingleRegenerationPartnerships.Thiswillenablethehospitaltoplayitspartina‘wholesystems’approachtotacklingtherootcausesofillhealthanddeprivation.5.2.3TheHospitalwillseektodevelopclosecollaborativerelationshipswiththelocalPrimaryCareGroupsinordertoensurethatthehospitalprovidesappropriateservicesinwayswhichbestmeettheneedsoflocalpeople.InadditiontheHospitalwillseektodevelopcloserrelationshipsandjointworkingwithotherorganisationssuchasthelocalCommunityHealthCouncil(CHC)LocalAuthoritiesandsocialservices.TheHospitalwillwhereappropriateseektoworkinpartnershipwithvoluntaryorganisationsortheprivatesectortohelptoachieveitsoverallobjectives.5.2.4TheHospitalwillaimtoimprovecommunicationandconsultationprocesseswithlocalpeoplethroughcloserworkingwiththelocalCHCcommunitygroupsthelocalmediaandbyusingpatientsurveysandfocusgroups.5.2.5TheHospitalwillseekthelocalimplementationofthenationalInformationManagementandTechnologyStrategy(NHSExecutive1998InformationforHealth)toenablefasterandmoreaccessiblesharingofinformationandtheprovisionofmoretimelyandaccurateinformationonthequalityandaccessibilityofservices.5.3Toachievecontinuousimprovementinemploymentpracticesandtobeagoodemployerrecruitingandmaintainingawelltrainedandmotivatedworkforce.5.3.1TheHospitalwillseektoensurethathospitalemploymentpoliciesandfacilitiesenablethecontinuingrecruitmentandretentionofappropriatelyqualifiedstaffinsufficientnumberstoprovidehighqualityservices.5.3.2TheHospitalwillseekcontinuousimprovementintheprovisionoffamilyfriendlyemploymentpracticeswhichrecognisealltheneedsofourworkforce.5.3.3TheHospitalwillseektomaintainthe‘InvestorsinPeople’andthe‘2ticks’Disabilityawardstatuswhichithasalreadysecuredandwillfurtherenhancetrainingandcareerdevelopmentfacilitiesandsupportforallstaff.5.3.4TheHospitalwillcontinuetoapplyanequalopportunitiespolicytoemploymentpractices.Itwillaimtosecureaworkforcethatwherepossibleisrepresentativeofthelocalcommunityandwillseektopreventdiscriminationofanykind.5.3.5TheHospitalwillseektoachievecontinuousdevelopmentofgoodcommunicationsandstaffinvolvementthroughouttheorganisationstimulatinginnovationinbothpatientcareandmanagementpractices.5.3.6TheHospitalwillworktowardsthelocalimplementationoftheNationalHumanResourcesStrategy(DepartmentofHealth1998MakingitHappen).5.4Toprovideanenvironmentwithinandaroundthehospitalwhichispleasantaccessiblesafewellmaintainedcosteffectiveandsustainable.5.4.1TheHospitalwillcontinuetoconsolidatemaintainandimprovethehospitalsbuildingsandinfrastructureinordertoprovideasafeandpleasantenvironmentforthepatientscarersandstaffwhousethehospital.Asmentionedinsection5.1.2thePhaseIIIdevelopmentplanfortheSouthWingsitewillenabletheolderhospitalaccommodationtobeupgradedandsomeservicesmovedtonewerrefurbishedareas.5.4.2TheHospitalwillundertakeadetailedenvironmentalauditofthehospitalanditsuseofresources.Thiswillbewithaviewtodevelopinganenvironmentalstrategythattakesanintegratedapproachtoenvironmentalmanagementreducinglevelsofwasteandpollutionandusinglessnaturalresources.5.4.3TheHospitalwillseektoreviewaccessbothtoandwithinthehospitalsiteforpeoplewithadisabilitypedestrianscyclistsandthoseusingpublictransportItwillseektoworkwithotheragenciestoimproveoverallaccesstothehospital5.5TomaintainstrongcorporategovernancewithintheHospitalandensurefinancialsecuritywithinthecontextofserviceagreementssecuredwithPrimaryCareGroups.5.5.1TheHospitalwillseektomaintainfinancialsecuritybyworkingwithPrimaryCareGroupstosecurelongtermserviceagreementswhichensureanincomelevelthatissufficienttofundtheserviceswhichthehospitalprovidesandthepressureswithwhichitisfaced.5.5.2TheHospitalwillmaintainastronglevelofcorporategovernanceandcontrolsassurance.ItwillensurethatriskmanagementstrategiesareinplacetocopewiththechangingenvironmentwithinwhichtheHospitalfindsitselfprovidingcare.5.5.3TheHospitalwillcontinuetomaximisesourcesofotherincomeinordertosupportfurtherservicedevelopmentsandtheprovisionofhealthcaretolocalpeople.
