士或者博士。
Note:
1. All courses should be placed on one sheet, including the courses scored “0” or recorded as
“Absent”.
2. Students who have successfully defended their graduation thesis can fill in the blank next
to “Grade Adjudged:” with “Pass”; “Date of Grant” should be filled in with the same
date as written on your degree certificate; there is no need to add other words in the
“Degree Granted” blank .
3. Do not change the format or configuration of the above form.
