....................................................... 6 2 1 目的 为了规范中煤集团鄂尔多斯能源化工有限公司工程竣工文件归档与整理，特制订本规定。 2 适用范围 本规定适用于参与中煤集团鄂尔多斯图克化肥项目工程建设的所有参与方，包括业主、监理单位、工程质量监督机构、EPC承包商、施工承包商等。 3 职责 3.1 工程项目竣工文件的收集、整理、归档和移交应与项目的立项准备、建设和竣工验收同步进行。项目档案应完整、准确、系统。 3.2 公司档案室负责组织、协调和指导承包商、供应商编制工程项目竣工文件。 3.3 承包商、供应商应根据要求，完成各自职责范围或 合同 劳动合同范本免费下载装修合同范本免费下载租赁合同免费下载房屋买卖合同下载劳务合同范本下载 规定的竣工文件的编制和项目文件整理、归档工作。 4 工程竣工文件的收集 4.1 项目准备阶段 ? 设计单位负责收集、积累勘察、设计文件，并按规定向建设单位档案部门提交有关设计基础资料和设计文件。 4.2 项目施工阶段 ? 由各分包商负责其分包项目全部文件的收集、积累、整理，并提交总承包商汇总;项目监理单位负责收集、积累项目监理文件。 ? 业主委托的项目监理单位负责监督、检查项目建设中文件收集、积累和完整、准确、系统情况，审核、签署竣工文件，并向业主提交有关专项报告、验证材料及其他监理文件。 4.3 收集范围 项目文件归档范围和保管期限见附录A。 工程竣工文件质量要求 5 5.1 标准 excel标准偏差excel标准偏差函数exl标准差函数国标检验抽样标准表免费下载红头文件格式标准下载 和规范 DA/T 28-2002 凡为易褪色材料(如复写纸、热敏纸等)形成的并需要永久和长期保存的文件，应附一份复印件。 5.5 复印、打印文件及照片的字迹、线条和影像的清晰及牢固程度应符合设备标定质量的要求。 5.6 录音、录像文件应保证载体的有效性，要有完整、简明的文字说明。 5.7 长期存储的电子文件应使用不可擦除型光盘，并且必须形成相应的纸质文件归档。 6 工程竣工文件的编制 6.1 竣工文件编制要求 ? 项目施工及调试完成后，施工单位、监理单位应根据合同规定的要求编制项目竣工文件。 ? 竣工文件由施工单位负责编制，监理单位负责审核签认。文件要求用A4 纸打印;提交正本一份,副本三份。 ? 对施工文件、施工图及设备技术文件的准确性和更改情况进行核实，并按要求修改或补充标注到相应的文件上。 ? 与施工图有关的设计变更、现场洽商和材料变更(复印件)应与竣工图编在一起(原件单独组卷)。 6.2. 竣工图编制要求 ? 项目竣工时要编制竣工图。项目竣工图应由施工单位负责编制，监理单位负责审核并签字。 ? 竣工图应完整、准确、清晰、规范、修改到位，真实反映项目竣工验收时的实际情况。 ? 按施工图施工没有变动的，由竣工图编制单位在施工图上加盖并签署竣工图章。 ? 一般性图纸变更及符合更改要求的变更，可在原图上更改，加盖并签署竣工图章。 ? 涉及结构形式、工艺、平面布置、项目等重大改变及图面变更面积超过35%的，应重新绘制竣工图。重绘图按原图编号，注明版本，在新图图标内标注“竣工阶段”，加盖并签署竣工图章。 ? 同一建筑物、构筑物重复的标准图、通用图可不编入竣工图中，但应在图纸目录中列出图号，指明该图所在位置并在编制说明中注明;不同建筑物、构筑物应分别编制。 ? 竣工图图幅应按GB/T 10609.3 要求统一折叠。 ? 编制竣工图总说明及各专业的编制说明，叙述竣工图编制原则、各专业目录及编制情况。 6.3 竣工图的更改方法 ? 文字、数字更改一般是杠改;线条更改一般是划改;局部图形更改可以圈出更改部位，在原图空白处重新绘制。 ? 利用施工图更改，应在更改处注明更改依据文件的名称、日期、编号和条款号。 4 ? 无法在图纸上表达清楚的，应在标题栏上方或左边用文字说明。 ? 图上各种引出说明应与图框平行，引出线不交叉，不遮盖其它线条。 ? 有关施工技术要求或材料明细表等有文字更改的，应在修改变更处进行杠改，当更改内容较多时，可采用注记说明。 ? 新增加的文字说明，应在其涉及的竣工图上作相应的添加和变更。 6.4. 竣工图章的使用 ? 竣工图章内容、尺寸如图1 所示。 竣工图 施 工 单 位 编 制 人 审 核 人 技术负责人 编 制 日 期 监 理 单 位 总 监 现 场 监 理 单位为毫米，总宽80毫米(每列宽20毫米)，总高50毫米(最下5行为7毫米)。 图1 竣工图章 ? 所有竣工图应由编制单位逐张加盖并签署竣工图章。竣工图章中的内容填写应齐全、清楚，不得代签。 ? 竣工图章应使用红色印泥，盖在标题栏附近空白处。 .5 竣工图的审核 6 ? 竣工图编制完成后，监理单位应督促和协助竣工图编制单位检查其竣工图编制情况，发现不准确或短缺时要及时修改和补齐。 ? 竣工图内容应与施工图设计、设计变更、洽商、材料变更、施工及质检记录相符合。 ? 竣工图以单项工程为归档单位，按单位工程或专业编制，并配有详细编制说明和目录。 ? 编制竣工图时应使用新的施工图，按要求由编制单位逐张加盖并签署竣工图章。竣工图章中的内容填写应齐全、清楚、不得代签(编制单位、监理单位、技术人员签字)。 ? 一张更改通知单涉及多图的，如果图纸不在同一卷册的，应将复印件附在有关卷册中，或在备考表 5 中说明。 ? 国外引进项目、引进技术或由外方承包的建设项目，外方提供的竣工图应由外方签字确认。 6.6 竣工图套数 提交的项目竣工图及交工技术文件纸质文件为四套、电子版四套，由施工单位向业主移交。 6.7 编制竣工图的费用 ? 编制竣工图所需的费用，由施工单位承担。 ? 因修改需重新绘图的，除合同规定外，应由设计单位负责绘制新图的费用。 6.8 合同要求 ? 设计合同:规定应提交的施工图套数并预留四套施工图用以编制竣工图，因修改需重新绘图的，应由设计单位负责绘制新图。不按要求执行的应令其改正或酌情扣减合同价款。 ? 施工合同:竣工文件(含竣工图)由施工单位负责编制和提交，监理单位负责审核。 项目档案的名称:——项目(第一层) ——单项工程名称(第二层) ——单位工程或专业名称(第三层) ——第X 卷 第X 册(第四层) 内容:见《国家重大建设项目文件归档要求与档案整理规范》附录A。 版本:最终版 套数:一套(正本)三套(副本)其中副本签字(手签)盖红章，监理负责监督审查。 时间:竣工验收后三个月内 费用:施工单位承担 违约责任:施工单位不执行上述规定的不予结算工程款，对于不完全执行的应酌情扣除工程质量保证金。 总平面的编制: 业主委托有资质的单位编制项目总平面图和综合管线竣工图。 ? 监理合同: 监理单位对竣工文件负有审核和向业主提交监理档案的责任。 7 工程竣工文件的整理与归档 7.1 项目文件的整理 ? 建设项目所形成的全部项目文件在归档前应根据国家有关规定，并按业主档案管理的要求，由文件形成单位进行整理。 ? 勘察、设计单位形成的基础材料和项目设计文件，应按项目或专业整理。 6 ? 施工技术文件应按单项工程的专业、阶段整理;检查验收记录、质量评定及监理文件按单位工程整理。 ? 设备、技术、工艺、专利及商检索赔文件应由承办单位整理;现场使用的译文及安装、调试形成的非标准图、竣工图、设计变更、试运行及维护中形成的文件、工程事故处理文件由施工单位整理。 7.2 项目文件的组卷 ? 组卷要遵循项目文件的形成规律和成套性特点，保持卷内文件的有机联系;分类科学，组卷合理;法律性文件手续齐备，符合档案管理要求。 ? 项目竣工图按公司建设项目档案分类编号规则组卷: ? 项目施工文件按单项工程、单位工程/装置、专业组卷; ? 设备文件按专业、台件等组卷;管理性文件按问题、时间或项目依据性、基础性、竣工验收文件组卷; ?监理文件按文种组卷;原材料试验按单项工程、单位工程组卷。 7.3 案卷与卷内文件的排列 ? 管理性文件按问题、时间或重要程度排列。 ? 施工文件按管理、依据、建筑、安装、检测实验记录、评定、验收排列。 ? 设备文件按依据性、开箱验收、随机图样、安装调试和运行维修等顺序排列。 ? 竣工图按专业、图号排列。 ? 卷内文件一般文字在前，图样在后;译文在前，原文在后;正件在前，附件在后;印件在前，定(草)稿在后。 7.4 案卷编目 ? 卷内文件页号的编写 卷内文件有书写内容的页面均应编写页号，页号编写位置:单面书写文件在右下角;双面书写文件，正面在右下角，背面在左下角;图样的页号编写在右下角，或标题栏外右上方。成套图样或印刷成册文件，自成一卷的，原目录可代替卷内目录，不必重新编写页号;与其它科技文件材料组成一卷的，应排在卷内文件材料最后，将其作为一份文件填写卷内目录，不必重新编写页号，可在备注中注明总页数。各卷之间不连续编页号。卷内目录、卷内备考表不编写页号。 ? 卷内目录的编制(见附录B 承包商归档文件格式要求) ? 案卷封面和脊背的编制(见附录B 承包商归档文件格式要求) ? 卷内备考表(见附录B 承包商归档文件格式要求) 7.5 案卷装订要求 ? 文字材料可采用整卷装订形式;图纸应按GB/T 10609.统一折叠。 7 ? 案卷内不应有金属物。 ? 案卷内不同幅面的科技文件材料要折叠为统一幅面，破损的要先修复。幅面采用国家标准A4型 (297mm×210mm)，图样折叠时标题栏露在右下角。 ? 外文资料应保持原来的案卷及文件排列顺序、文号及装订形式。 7.6 卷皮、卷内表格规格及制成材料 (见附录B 承包商归档文件格式要求) 7.7 声像材料整理 声像材料整理时应附文字说明，对事由、时间、地点、人物、作者等内容进行著录。 7.8 项目文件的归档 ? 各承包商应在建设项目完成后，将经整理、编目后所形成的项目文件按合同协议规定的要求，向业主档案管理机构归档。 ? 根据基本建设程序和项目特点，归档在单项工程完成并通过竣工验收后与竣工验收文件一并归档。 ? 归档文件应完整、成套、系统。应记述和反映建设项目的规划、设计、施工及竣工验收的全过程;真实记录和准确反映项目建设过程和竣工时的实际情况，图物相符、技术数据可靠、签字手续完备;文件质量应符合本文中项目文件质量要求。 ? 外文资料应将题名、卷内章节目录译成中文;经翻译人、审校人签署的译文稿与原文一起归档。 7.9 项目文件的归档审查 ? 承包商在项目竣工文件收集、编制和整理后，应依次由竣工文件的编制方、质监部门、监理部门对文件的完整、准确情况和案卷质量进行审查或三方会审;经业主单位确认并办理交接手续后连同审查记录全部交业主存档。 7.10 项目文件的归档时间 ? 在单项工程完工后三个月内，承包商(分包商)应将各自的项目文件汇总整理，将项目文件向业主归档。 7.11 竣工档案的整理 ? 项目档案的汇总整理在业主的组织下根据公司建设项目档案分类编号规则进行。 ? 依据公司建设项目档案分类编号规则对全部项目档案进行统一的分类和编号(承包商可用铅笔临时填写档案号)。 ? 对全部项目档案进行清点、编目，并编制项目档案案卷目录及档案整理情况说明。 ? 执行国家及本行业的技术规范及各种技术文件表格。 7.12 项目档案的移交 ?
