主诉:阴道不规则出血 19 天,伴发作下腹痛三次
Chief complaint:Irregular vaginal bleeding for 19 days, with episodes of lower abdominal pain three times
现病史:患者平素月经规律,末次月经为 2003 年 4 月 20 日,7 天干净,量、现病史色同以往。5 月 9 日起阴道少量出血,暗红色。5 月 11 日突感下腹较重的疼痛,阵发性,并有鲜红色阴道出血,量同月经,可见有肉样组织物排出后,腹痛减轻,前后持续 1 小时。以后一直有少量阴道出血。5 月 27 日上午再次突感下腹阵发性疼痛,较剧烈,伴恶心、不吐,出冷汗、头晕,无肛门坠胀,持续 2~3 小时缓解。中午曾去妇产医院就诊,按抗炎治疗。5 月 28 日晨 1~5 时第三次类似发作,腹痛较前两次重,但无恶心、吐。当晚 11 时即来我院就诊。自发病来,精神饮食差,睡眠欠佳,二便正常。
History of present illness:Patients menstruation is usually regular and the last menstrual period is April 20 to 27, the amount and the color is familiar with the past. On May 9th, the vaginal started to bleed, the color is dark red. On May 11th , patient suddenly felt severe and paroxysmal pain in lower abdomen with bright red vaginal bleeding, the amount is the same as menstruation. After discharging the meat like tissue, the abdominal pain was relieved. The situation lasted for one hour. In the future, there had been a small amount of vaginal bleeding for several times. Again on the morning of May 27th , the patient suddenly felt severe and paroxysmal pain in the lower abdomen with nausea, vomit, cold sweats and dizziness but no anal bulge and lasted for 2 to 3 hours. Patient went to the maternity hospital at noon, receiving the anti-inflammatory treatment. In the morning of May 28 from 1a.m. to 5a.m., the third similar pain attacked with more severe abdominal pain than the previous two times, but without nausea, vomiting. At 11:00 that night, patient came to our hospital. Since the onset, the spirit, diet and sleep is poor. Feces and urine are normal.
