——每一项服务特性的验收标准。 1 DB11/T 304—2005 3.2 服务提供规范 是对描述服务提供过程所用方法的服务提供程序，包括: ——对直接影响服务业绩的服务提供特性清晰的阐述; ——每一项服务特性的验收标准; ——设备、设施的类型和数量的详细资源要求应是为实现服务规范所需的; ——所需人员的数量和技能; ——对提供产品和服务的分供方的可信赖程度。 3.3 服务质量控制规范 是服务过程(开发、设计和服务提供)的一个组成部分，应有效地控制每一服务过程，以确保服务始终满足服务规范和顾客需要。 质量控制设计包括: ——识别每个服务过程中对规定的服务有重要影响的关键活动; ——对关键活动进行分析，选出一些保证服务质量的特性加以测量和控制; ——建立在规定界限内影响或控制特性的手段。 3.4 危机 是一个会引起潜在负面影响的具有不确定性的大事件，这种事件及其后果可能对组织及其人员、产品、服务、资产和声誉造成巨大损害。 4 标准体系的基本要求 4.1 技术标准体系、管理标准体系和工作标准体系均应在DB11/T 303的框架下制定，体系表应符合GB/T 13017的要求。 4.2 技术标准体系是管理标准体系和工作标准体系的重要依据。管理标准体系应能保证技术标准体系的实施。工作标准体系应确保技术标准体系和管理标准体系的实现。构成技术标准体系、管理标准体系和工作标准体系的标准之间应相互协调一致，标准的各项要求应系统、协调、统一、切实可行。 4.3 标准的存在形式可以是标准、规范、规章、守则、流程图、作业指导书等。 4.4 应充分吸收和运用国内外先进管理理论和经验，结合实际，将其应用在管理标准体系的建立和实施中。 4.5 本标准规定的要求是通用的。不同性质的养老服务机构可根据提供服务的种类和特点适当增减、归并。增减、归并不能影响养老服务机构提供服务与适用法律法规责任的要求和标准体系的系统性和有效性。 5 技术标准体系 5.1 概述 构成技术标准体系的标准包括养老服务机构所采用和贯彻的国家标准、行业标准、地方标准及本机构制定的技术标准。所有技术标准均应在标准化法律法规、各种相关政策和本机构制定的方针、目标等指导下形成有机整体。 5.2 技术标准体系的层次结构 养老服务机构编制技术标准体系时，可采用层次结构图表示，见图1。 2 DB11/T 304—2005 技术基础标准 危 机 采能环设服与职设信 购源境计务风业施息 技技技技技险健设技图1 术术术术术控康备术 标标标标标制技技标 准 准 准 准 准 技术术准 术标标 标准 准 准 注1:本体系表中的各类技术标准均表示养老服务机构内服务提供涉及到的该类技术标准的总和。 注2:层次结构的技术标准体系第一层是技术基础标准，其覆盖面是养老服务机构的服务标准、服务实现过程中所 有综合性的技术基础标准。技术基础标准是指导养老服务机构服务标准和提供服务实现过程中技术标准制定的基 础。 注3:第二层的技术标准是服务提供过程中主要的技术标准。 图1 技术标准体系的结构形式 5.3 技术基础标准 5.3.1 概述 技术基础标准是在一定范围内作为其他标准的基础，并普遍使用，具有广泛指导意义的标准。养老服务机构技术基础标准是在养老服务机构范围内，作为本机构制定技术标准、管理标准、工作标准的基础。 5.3.2 技术基础标准内容 5.3.2.1 标准化工作导则 标准化工作导则是指导养老服务机构开展标准化工作的指导性文件。包括: a) 标准的结构和编写规则，如GB/T 1.1; a) 标准编写规则，如GB/T 20001; b) 标准中规范性技术要素内容的确定方法，如GB/T 1.2的有关规定; c) 各类标准的通用编写规定，如GB/T 1.1的有关规定; d) 当GB/T 1.1不能满足养老服务机构特殊需求时，可制定本机构标准编写规定; e) 体系文件编号规则应符合DB11/T 303的要求。 5.3.2.2 术语和定义标准 按照GB 15497中 7.2.2执行。 5.3.2.3 量和单位标准 按照GB15497中 7.2.3执行。 5.3.2.4 数值和数据标准 按照GB15497中 7.2.4执行。 5.3.2.5 公共信息图形符号 按照GB/T 10001.1相关要求执行。 3 DB11/T 304—2005 5.3.3 技术基础标准的编写方法 5.3.3.1 收集 养老服务机构应收集下列有关技术标准: a) 全国通用的综合性标准; b) 行业通用标准; c) 地方通用标准。 5.3.3.2 转化 养老服务机构对技术基础标准一般可直接引用，也可根据本机构需要，选择其中部分内容转化为本机构标准或规定，供机构内部使用。 5.3.3.3 编入养老服务机构技术标准体系的方法 国家标准、行业标准中的标准编入养老服务机构标准体系的方法是: a) 对于国家、行业的标准，可先将其编号和名称集中编入一个标准，然后再将标准文本编入技术 标准体系; b) 将国家、行业的标准编号和名称直接写进标准体系中，然后再将标准文本编入技术标准体系。 5.3.4 技术基础标准制定要求 应建立齐全、有效的养老服务机构技术基础标准，对制定技术标准有指导和制约作用。能够指导服务提供技术的国家或行业基础标准应纳入技术基础标准体系。没有国家或行业基础标准的，养老服务机构可自行制定服务提供技术基础标准。 5.4 设计技术标准 5.4.1 概述 设计技术标准是为保证和提高服务产品设计质量而制定的标准。 5.4.2 内容: a) 设计符号、代号、术语标准; b) 产品安全、环境和其他法规要求及国际惯例; c) 专业设计规范; d) 设计参数与数据标准; e) 设计中用于评价服务过程的质量方法和规则; f) 设计图样与文件的格式和编号标准; g) 设计文件完整性要求。 5.4.3 制定要求: a) 设计标准应满足适用的法律、法规和强制性标准的要求; b) 服务设计标准应满足顾客的需求,服务的功能和特性要求; c) 在设计标准中对重要质量特性应做出明确而适当的规定，保证服务实现得到有效的控制; d) 明确服务规范、服务提供规范、服务质量控制规范的各项要求。 5.5 采购技术标准 5.5.1 概述 采购技术标准是指对养老服务机构服务提供过程中所需的外包服务、外购食品、生活用品、燃料、设施设备、医疗护理用品等的质量要求制定的标准。 5.5.2 内容: a) 采购质量要求应规定采购物品适用的质量标准(如质量特性、品种、规格、等级)，采购质量 要求可以引用国家标准、行业标准、地方标准，也可以直接规定具体技术要求; b) 验收规则:对外包服务、采购物资应规定验收规则，检查外包服务、采购物资的质量是否符合 规定的质量要求及其购买条件。验收项目可根据需要与具备的技术条件，规定验收的检测项4 DB11/T 304—2005 目、数量、频次等; c) 验收方法:对验收的项目应规定相应的检验方法; d) 保管注意事项:主要规定采购物资的保管和贮存期限，及对不合格采购物资隔离存放的规定， 并有记录要求。 5.5.3 制定要求: a) 采购标准应保证对外包服务、采购物资进行严格控制，防止不合格的服务和物资进入; b) 确定验收规则时，应考虑其成本和对服务质量的影响; c) 要规定对采购物资品种、规格的优化、压缩及选用原则。 5.6 服务技术标准 5.6.1 概述 服务技术标准是指对服务提供过程所涉及的技术事项而制定的标准。 5.6.2 内容: a) 服务质量标准，是指服务的提供以及对服务的检验、验收和仲裁检验的依据，是养老服务机构对 顾客和社会的服务质量责任承诺; b) 服务质量控制技术规范; c) 对服务及服务提供发生的质量缺陷进行处置的技术规范。 5.6.3 制定要求: a) 服务质量标准应是符合《中华人民共和国标准化法》要求的合法标准，服务质量标准可执行 DB11/T 148; b) 服务质量标准内容应满足顾客的需求，顾客虽然没有明示，但服务质量标准内容也应满足规定 的服务功能或已知的预期功能所必需的要求; c) 应规定特定的服务和服务提供应达到的水平和要求。如结合服务项目的特点规定定量和定性质 量指标要求，一般包括服务的功能性、经济性、安全性、舒适性、时间性和文明性的要求; d) 服务质量控制技术规范应规定控制的内容、控制程序和方法; e) 养老服务机构其他服务技术标准可参照以下标准和规范要求:DB11/T 149、DB11/T 219、DB11/T 220和民政部《老年人社会福利机构基本规范》等的技术要求。 5.7 能源技术标准 5.7.1 概述 能源技术标准是指对用能和节能所涉及的技术事项而制定的标准。 5.7.2 内容: a) 能源产品标准:包括煤、电、油、气体燃料、热力、水等产品质量标准; b) 能源设备及其系统的经济运行标准，包括:能源的转换设备、供能输送设备、用能终端设备的 耗能定额标准，以及设备经济运行规范等; c) 省能材料标准，包括:载能、隔能材料及能量传导材料技术标准。 5.7.3 制定要求: 应贯彻国家、行业和地方节能法律、法规和能源标准。根据实际制定本机构能源技术标准，为节能管理提供技术依据。 5.8 危机与风险控制技术标准 5.8.1 概述 危机与风险控制技术标准是指以保护生命和财产安全为目的而制定的技术标准。 5.8.2 内容: 涉及提供养老服务全过程中安全通用标准、院内感染控制、各种事故的防范、突发事件的处理、日常服务监控、设施设备安全标准等方面，包括: 5 DB11/T 304—2005 a) 安全通用标准:包括安全标志和报警信号标准、危险因素分类技术标准; b) 院内感染控制标准:包括传染病的预防技术和院内感染控制技术标准; c) 各种事故的防范规范:包括食物中毒、火灾、医疗事故等防范规范; d) 突发事件分类技术标准:各种突发性疾病抢救标准和突发事件处理预案; e) 养老服务安全保护技术标准; 设施设备安全标准:电器、压力容器、锅炉、电梯等特种设备使用安全标准; f) g) 无障碍基础设施标准和建筑基本规范。 5.8.3 制定要求: a) 应将国家和行业有关的安全基础标准、服务质量、安全保护技术和危机控制技术要求纳入危 机与风险控制技术标准; b) 需要顾客注意的危机与风险控制技术要求应纳入到本机构服务提供过程中、维修手册、标签 及宣传材料; c) 使用适宜的院内感染的监测方法、控制技术; d) 根据实际制定相关预案和技术操作规程，为安全服务、危机控制、防范事故提供技术依据。 5.9 职业安全技术标准 5.9.1 概述 职业安全技术标准是指为消除、限制和预防职业活动中的健康损害和安全危险及其有害因素而制定的标准。 5.9.2 内容: a) 空气、噪声、温湿度限量标准等; b) 有害物质浓度限量标准; c) 感染控制标准等; d) 劳动防护技术标准。 5.9.3 制定要求: a) 相关法律、法规和其他法规技术要求; b) 服务过程中涉及职业安全危险源的有害因素; c) 对服务场所和操作现场的职业安全要求; d) 职业安全要求的监测方法和评价方法; e) 控制及防护措施等。 5.10 环境技术标准 5.10.1 概述 环境技术标准是指为保护环境和有利于生态平衡，对环境质量、监测方法及其他事项制定的标准。 5.10.2 内容: 涉及医疗废弃物、废气、生活垃圾等处理的技术标准。 5.10.3 制定要求: 国家和地方环境有关技术标准是强制性标准，应依法执行。 5.11 信息技术标准 5.11.1 概述 信息技术标准是指对养老服务机构内各种信息的收集、贮存、传递、利用等信息技术活动制定的技术标准。 5.11.2 内容: 包括养老服务机构内各种信息的收集、贮存、传递、利用等信息技术活动。 5.11.3 制定要求: 信息技术标准应满足有关适用的法律、法规和标准的要求。将养老服务机构的全部标准文件和记6 DB11/T 304—2005 录处于受控状态和便于应用。 5.12 设施设备技术标准 5.12.1 概述 设施设备技术标准是指根据相关标准要求对设施设备的技术条件及维修保养质量要求而制定的标准。 5.12.2 内容: a) 设施设备的技术标准; b) 维修保养规程; c) 设备操作规程。 5.12.3 制定要求: 应贯彻国家、行业和地方法律、法规和标准。根据实际制定设施设备维修保养规程和操作规程，为安全运营提供技术依据。 6 管理标准体系 6.1 管理标准体系的构成 6.1.1 管理标准体系包括养老服务机构所执行的国家标准、行业标准、地方标准和养老服务机构制定的管理标准。 6.1.2 养老服务机构的管理标准体系可以是一个独立的体系，也可以是多个管理标准体系组合形成，但应避免重复和矛盾。 6.1.3 管理标准体系的结构形式见图2。本结构形式图的管理内容、类别基本涵盖了一般养老服务机构的主要管理活动。 6.2 管理标准体系的层次结构 6.2.1 管理标准体系采用层次结构。 6.2.2 同层次的技术标准与管理标准应确保相互协调。 管理基础标准 设 危 计采能环服机信职设综标 、体购源境务与息业施合准开 系管管管管风管安设管化评发 理理理理险理全备理管与价 标标标标控标管管标理创管 准 准 准 准 准 制准 理理标新理 管标标准 管标 理准 准 准 理 标标 准 准 图2 管理标准体系的结构形式图 6.3 管理标准的编写要求 6.3.1 名称 7 DB11/T 304—2005 标准名称应简明、确切，反映管理活动的主 题 快递公司问题件快递公司问题件货款处理关于圆的周长面积重点题型关于解方程组的题及答案关于南海问题 。标准名称除采用“管理标准”以外，也可以采用管理活动的主题与“程序”、“控制”、“办法”、“制度”等的组合，例如:合同评定程序、采购控制、能源管理办法、财务 管理制度 档案管理制度下载食品安全管理制度下载三类维修管理制度下载财务管理制度免费下载安全设施管理制度下载 等。 6.3.2 职责 明确主管部门实施此项管理部门及他们的职责、权限。该管理活动涉及几个部门时，应规定主管部门、协作部门及其接口和相互关系，同时应明确该项标准贯彻、实施、检查、考核的部门及方法。 6.3.3 管理活动的内容与方法 养老服务机构在制定管理标准时，首先确定管理内容和方法，包括: a) 应详细规定该管理活动所涉及的全部内容和应达到的要求、采取的措施和方法; b) 逐步列出开展此项活动的细节，明确输入、转换的各环节和输出的内容。其中包括人员、信息 和环境方面应具备的条件，与其他活动接口的协调措施; c) 明确每个服务过程中各项工作内容、定人、定点、定时、定量进行控制; d) 内容复杂的管理标准，可根据管理活动的特点或类别分别列出若干章节，分别表述; e) 管理要求要尽可能量化，不能量化的可使用比较的方式表述。 6.3.4 报告和记录 养老服务机构管理活动所形成的报告、记录，其格式、签发、传递和保存应符合国家、行业、地方的要求，若无明确要求的，应制定具体要求。 6.4 管理标准的构成及要求 6.4.1 管理基础标准 管理基础标准是养老服务机构制定各项管理标准的基础，可以包括: a) 管理术语; b) 服务标准化指南。 6.4.2 综合管理标准 6.4.2.1 方针目标管理 养老服务机构各项工作应在本机构方针指导下进行，方针和目标应与上级部门的方针和目标保持一致。 在制定方针目标时应考虑: a) 方针应在本机构内得到沟通和理解; b) 目标应是可测量的，应通过检验以确定服务实现的程度; c) 在相关职能和层次上展开目标时注意各部门之间配合及协调关系，确保方针目标的实现。 6.4.2.2 合同管理 合同管理标准应包括合同管理的程序、内容和相关的要求。建立合同管理标准要求: a) 规范合同格式，确认格式等效性; b) 明确合同授权或委托权限; c) 合同管理依据《中华人民共和国合同法》有关规定; d) 合同评审应依据DB11/T148 中4.2的规定。 6.4.2.3 财务管理 应按国家法律法规和行业的要求，对财务活动和成本核算以及收支进行管理，并制定财务成本收支标准。 管理者应支持和鼓励养老服务机构的业绩改进，开展具有创新性的财务管理方法。 6.4.2.4 人力资源管理 应包括:人员招聘、培训、业绩考核和聘用管理标准等。 编制人力资源管理标准要求: 8 DB11/T 304—2005 a) 执行国家和行业有关人力资源方面的职业要求; b) 建立培训制度时，应考虑养老服务机构发展、人员综合素质要求以及养老服务机构文化现状 的需要; c) 建立评价与考核制度，详细规定组织形式、考核内容、频次和奖惩办法; d) 强调养老服务理念更新，满足服务对象需求。 6.4.3 设计、开发与创新管理标准 6.4.3.1 养老服务机构应对设计开发进行控制，对改革创新提出要求，并制定管理标准。包括:设计开发输入输出及创新工作的要求，并进行评审、验证。 6.4.3.2 设计开发输入输出的要求: a) 设计开发输入应反映相关强制性标准、法律法规以及行业的要求、服务的安全性和舒适性的要 求; b) 设计开发输入应采集国际国内同行业的相关信息; c) 设计开发输出应注意服务提供过程的量化指标。 6.4.3.3 评审、验证的要求: a) 应对设计结果进行评审，验证设计要求，对收集的数据进行认定; b) 确定服务设计是否满足需求，并留有记录。 6.4.3.4 服务创新工作的要求: a) 管理者应主动寻求对服务管理的有效性和效率持续改进; b) 建立服务创新必要的工作程序并制定奖励办法。 6.4.4 采购管理标准 养老服务机构对采购活动进行控制，并制定采购管理标准。内容应包括: a) 在审批采购时，应加强对其准确性和完整性的审查; b) 标准应规定在采购过程中选择合格的供方。要求供方提供其作为合格供方的证据，包括:供 方的资质、产品、程序、过程、设备、人员的概况;以及供方的产品安全认证; c) 根据供方提供产品的能力，进行评价和选择; d) 对评价结果记录予以保存; e) 应建立并实施检查、验收，以确保采购的产品符合质量要求; f) 不合格品的处置。 6.4.5 服务管理标准 6.4.5.1 应对提供服务的人员、设施、资源、方法、程序和环境等因素及服务提供的全过程进行有效控制，制定服务管理标准。 6.4.5.2 内容: a) 制定服务规范，规定服务达到的水平和要求，包括:功能性、经济性、安全性、舒适性、时间 性、文明性的要求等; b) 制定服务提供规范，将服务提供过程按步骤划分成各个工作环节，规定每个工作环节中的内容。 应注意: 1)提供过程所要求的各项设备、设施、器材、人员等资源; 2)编制服务程序，确定各类服务过程中的接口，不应留有空白或空缺。 c) 制定质量控制规范，包括: 1) 识别并分析对服务质量有重要影响的关键过程，并加以控制; 2) 规定过程的评价方法及建立控制手段; 3)对不合格项的管理应制定标准，包括不合格项的原因分析、识别、记录、评审和处置等控制 办法，防止不合格项对养老服务机构造成损失;应制定纠正措施标准，以消除不合格项发生 9 DB11/T 304—2005 的原因，防止不合格项的再发生; 4)制定预防措施标准，以消除潜在的不合格的原因，防止不合格的发生。制定预防措施;对预 防措施进行监控，制定跟踪措施并评价; 5)质量争议的处置; 6)服务满意度调查。 6.4.5.3 服务管理标准应制定评价和改进程序，持续改进服务质量以满足顾客需求。 6.4.6 能源管理标准 6.4.6.1 养老服务机构对能源的使用和消耗的全过程进行科学的计划、监督和调节，并建立能源管理标准。 6.4.6.2 内容应包括: a) 在满足顾客需要的前提下进行节能调整和改造; b) 能源管理各种记录的保存和利用; c) 能源的计量和能耗的分析; d) 注意能源的合理使用，降低费用支出。 6.4.7 危机与风险控制管理标准 6.4.7.1 GB/T 28001 规定的管理手册、程序文件，是危机与风险控制管理标准的一种形式，可将其纳入养老服务机构管理标准体系。 6.4.7.2 管理过程应包括: 养老服务机构提供服务的全过程中的院内感染控制、各种事故的防范、突发事件的处理、日常服务过程监控、安全管理、人员培训等方面的管理。 6.4.7.3 内容应包括: a) 加强传染病的预防教育、院内感染控制培训、各类安全教育和各种技术操作培训; b) 明确检查制度，规定院内感染控制环节、各种事故的防范、突发事件的处理、日常服务过程监 控、安全管理、人员培训、设施设备、安全防护等方面的危机管理措施; c) 编制各类危机与风险控制的工作预案和处理程序; d) 通过风险评估、服务过程控制，采取预防和纠正措施并获得改进。 6.4.7.4 标准应明确处理程序，包括: a) 识别潜在的或紧急情况，及时做出反应，预防和减少可能的危险和伤害; b) 按照国家和行业规定进行事故分类;
