一、编制依据(包括但不限于以下法规及文件):

二、术语:
1、安全监理:
监理人员依据国家和地方有关法律、法规、工程建设强制性标准，对施工单位在施工现场安全生产管理行为的监督检查和安全防护措施的监督抽查。
安全监理工作的性质是发现安全问题，督促施工单位消除事故隐患。
监理单位安全监理不得替代施工单位的安全生产管理工作。
监理单位履行安全监理职责，不能替代和免除施工单位的安全生产主体责任，也不能替代和免除建设单位、勘察设计单位及其他各建设工程安全生产有关责任单位的安全生产责任。
建设单位在委托监理合同中，应明确安全监理的范围、内容、职责;同时将安全监理的委托范围、内容及对工程监理单位的授权，书面告知施工单位。
监理单位依据《建设工程安全生产管理条例》承担相应的监理责任。
2、安全监理人员:
按规定经有关部门安全教育培训，从事安全监理工作的监理人员。
3、安全生产管理机构:
施工单位及其在建设工程项目中设置的负责安全生产管理工作的独立职能部门。
4、施工单位专职安全生产管理人员:
取得安全生产考核合格证书，在施工单位从事安全生产管理工作的专职人员。包括施工单位安全生产管理机构的管理人员和施工现场专职安全生产管理人员。
5、重大危险源:
是指施工现场存在或潜在存在重大安全危险或隐患，如不采取有效防范措施有可能引发重大生产安全事故的分部分项工程和其意外的潜在不安全环境或因素。
三、监理单位安全监理的权力、责任及安全责任制:
(一)监理单位安全监理责任制:
1、经公司总经理授权，公司总工程师作为分管安全监理工作的领导对本公司承接工程项目的安全监理工作全面负责;
公司副总经理对所分管部门的安全监理工作负领导责任;
综合业务部代表公司对各事业部、分公司及项目监理部安全监理工作的组织、落实情况进行监督检查，根据对检查结果的汇总分析提出改进意见;
各事业部、分公司主要负责人负责本部门监理项目的安全监理工作的监督、检查和组织改进;
项目总监理工程师(或项目经理)对工程项目的安全监理工作负责，并根据工程项目特点，确定施工现场具体安全监理人员，明确其工作职责;
安全监理人员在总监理工程师的领导下，进行工程项目日常安全监理工作，安全监理人员应熟悉建设工程施工安全的相关法规，并具备相关的专业知识，在具备条件时，应接受安全监理业务教育培训，并通过相关考核。
(二)、监理单位履行安全监理职责拥有以下权利:
1 建议权:监理单位可以就工程在勘察、设计、施工工艺设计等方面，对影响安全施工的因素提出优化建议的权利。
2 审查认可权:监理单位对施工单位上报的施工组织设计及专项施工方案中的安全技术措施以及分包单位具有审查认可权。
3 指令权:监理单位有权发布开工令。因施工单位存在安全生产问题有权发布暂停令、复工令、限期整改指令等。
4 检查、监督权:监理单位对施工单位在施工过程中的安全防护设施和安全生产行为有检查、监督权。
5 签认权:监理单位对分部分项工程有验收签认的权利，对安全防护、文明施工措
施费用支付有审核签认权。
6 报告权:监理单位对施工过程中发生的安全问题有向建设单位和有关部门报告的权利。
7 要求权:监理单位对安全监理服务的报酬和合理条件有向建设单位提出要求的权利。
8 国家和地方法律、法规规定的其他权利。
(三)、监理单位安全监理的责任
1 监理单位应对施工组织设计中的安全技术措施或专项施工方案进行审查。施工组织设计中的安全技术措施或专项施工方案未经监理单位审核确认，施工单位擅自施工的，监理单位应及时下达工程暂停令，并及时将情况书面报告建设单位。
2 监理单位在监理巡视检查过程中，发现存在安全事故隐患的，应按照有关规定及时下达书面指令要求施工单位进行整改或停止施工。
3 施工单位拒绝按照监理单位的要求进行整改或停止施工的，监理单位应及时将情况向有关部门报告。
4 监理单位应依照法律、法规和工程建设强制性标准实施监理。
5 监理单位履行了上述职责，施工单位未执行监理指令继续施工或发生事故的，监理单位不承担法律责任。
(四)本项目监理范围:
整个建设项目施工全过程的监理:(包括但不限于)
土方、基坑支护、桩基、主体结构、保温节能、钢结构、人防、消防、安装(含强、弱电智能化)、通风、空调、电梯、综合管网、装修、景观、绿化、道路等工程施工至竣工验收、备案阶段监理的“三控三管一协调”等内容。
(五)本项目安全监理内容:
1 安全管理方面:
制订安全管理网络，督促施工单位完善安全机构及加强安全管理，每月定期、不定期举行安全检查，并将检查结果上报委托方，杜绝安全隐患，确保无安全事故。
保证驻现场工作人员身体健康，无不适合监理岗位工作的疾病;加强安全教育和配置必要的安全设施，确保监理人员的人身安全，并为其购买保险。
2 文明施工
督促承包人(施工单位)合理规划现场临时设施及材料堆放，督促施工单位严格按照“标化“要求管理，保持施工阶段现场整洁、文明、卫生。根据工程各阶段的要求，提前规划各阶段的工作，使得各阶段工作顺利衔接，促进施工各方提前做好相关的准备工作。

1、项目监理部岗位责任制度
(1)总监理工程师的职责
? 对所监理的工程项目安全监理工作全面负责;
? 确定项目监理部人员及各自的安全监理工作职责;
? 主持编写监理规划中有关的安全监理内容，并审批安全监理实施细则; ? 审核并签发有关安全的监理指令和安全监理专题报告;
? 审核确认施工组织设计和专项施工方案;
? 组织审核和确认施工单位提出的安全技术措施及工程项目安全事故应急救援预案;
? 签署《安全防护、文明施工措施费用支付证书》;
? 签发开工/复工报审表、工程暂停令，必要时向有关部门报告。 ? 检查项目监理部安全监理工作的落实情况;
? 审查施工单位的企业资质、安全生产许可证及安全生产保障体系情况; ?11 参与或配合本工程安全事故的现场调查。
(2)总监理工程师代表的职责
1) 根据总监理工程师的授权，行使总监理工程师的部分职责和权力，并承担相应的责任;
2) 总监理工程师不得将下列工作委托总监理工程师代表:
? 对所监理工程项目的安全监理工作全面负责;
? 主持编写监理规划中的安全监理方案，审批安全监理实施细则; ? 签署安全防护、文明施工措施费用支付证书;
? 签发安全监理专题报告;
? 签发开工/复工报审表、工程暂停令，必要时向有关部门报告; ? 组织审核和确认施工单位提出的安全技术措施及工程项目安全事故应急救援预
案;
? 检查项目监理部安全监理工作的落实情况;
? 调换不合格的监理人员。
(3)监理工程师的职责
? 编写安全监理实施细则;
? 核查项目经理和施工现场安全员的安全生产考核合格证书及特种作业人员的上岗资格证书;
? 审查施工组织设计及专项施工方案;
? 核查施工起重机械、整体提升脚手架、模板等自升式架设设施和安全设施的验收手续及使用期间的定期检测记录;
? 签发有关安全的监理指令;
? 向总监(代表)汇报有关安全问题和监理情况;
? 对施工现场进行安全巡视检查及安全专项检查，填写监理日志，记录有关安全监理检查情况;
? 编写安全监理专题报告。
(4)监理员的职责
? 在监理工程师的指导下开展安全监理工作;
? 填写监理日志，做好安全监理记录;
? 检查施工现场的安全状况，发现问题后及时要求施工单位整改，并向监理工程师或总监理工程师汇报;
? 做好安全专题会议的记录工作;
? 对施工过程中的危险性较大工程作业情况进行巡视;
? 督促施工单位做好安全培训教育及安全技术交底工作。
2、项目监理部会议制度
项目监理部根据相关规范建立了会议制度。会议纪要由项目监理部起草，经与会各方代表会签后分发与会各方。每次会议应符合下列要求:
——第一次工地会议。
下达本项目开工令之前召开。会议含有安全监理的内容:
(1)建设单位根据委托监理合同宣布对总监理工程师安全监理方面的授权;
(2)总监理工程师介绍负有安全监理职责的人员及其分工;
(3) 施工单位安全生产方面的施工准备情况(包括负责安全生产的人员及其分工、项目部安全保证体系建设等);
(4)明确必须召开安全监理专题会议的情况;
(5)约定安全监理交底会的时间、地点及参加人员。
会议由总监理工程师与建设单位的负责人联合主持召开。
——安全监理交底会。
下达本项目开工令之前或安全监理工作需要时召开。由总监理工程师或由其授权专职安全监理人员主持。参加安全监理交底会的人员应包括:施工单位项目经理、技术负责人及有关的安全管理人员;项目监理机构总监理工程师及有关的监理人员;建设单位有关的安全管理人员。
安全监理交底的主要内容:
(1)明确本工程适用的有关建设工程安全生产的法律法规和技术标准;
(2)介绍施工阶段安全监理工作的范围和内容;
(3)介绍施工阶段安全监理工作的基本程序、方法;
(4)提出有关施工安全资料报审及管理要求;
(5)项目监理机构接受施工单位有关安全监理工作的询问。
——监理例会。每周四下午2:30举行。由总监理工程师主持，参建各方项目经理、技术负责人以及有关的质量、安全管理人员参加。
会议中应含安全监理内容有:
(1)通报一周安全生产情况及周三安全检查情况;
(2)检查上次监理例会中有关安全生产决定的执行情况，分析当前安全生产存在的问题，提出问题的解决方案或建议;
(3)明确会后应完成的任务。
——安全监理专题会议。
安全监理工程师主持召开的安全监理专题会议，由总监理工程师或专职安全监理工程师主持，施工单位的项目负责人、现场技术负责人、现场安全管理人员及相关单位人员参加。专门研究解决施工中出现的涉及安全生产方面的问题。
3、安全培训教育制度
为不断提高各级监理人员在项目工程安全监理方面的业务水平和管理能力，公司根据上级有关文件精神，特制定本制度。
?、监理部在进驻项目工程开展工作前，应由总监理工程师和专职安全监理工程师组织对各级工程监理人员进行岗前安全培训;
?、监理部应定期组织监理人员进行安全岗前培训;
?、培训内容应包括本工程有关的法律、法规、安全工作规程、规定、制度等。培训时间每次不得少于8小时。培训结束后，应通过相应的考试，合格后方可持证上岗。
?、监理部应保留监理人员接受教育、培训、考试的记录。
?、总监理工程师和专业安全监理工程师的安全教育、培训，由公司工程管理部组织进行。
4、审查核验制度
根据现行法律法规规范要求，项目监理部建立了审查核验制度。
? 发现勘察、设计文件有不符合工程建设强制性标准及其他相关规定，或存在较大施工安全风险时，应向建设单位提出。
? 核查施工总承包单位、专业工程分包单位和劳务分包单位(以下简称分包单位)的企业资质和安全生产许可证，检查施工总承包单位与分包单位的安全

签订情况。
? 检查施工单位施工现场安全生产保证体系，主要内容包括:
? 施工现场安全生产组织机构;
? 施工现场安全生产规章制度;
? 施工单位项目负责人的执业资格证书和安全生产考核合格证书应齐全有效;
? 施工单位专职安全生产管理人员的配备数量应符合有关规定，其执业资格证书和安全生产考核合格证书应齐全有效。
? 审查施工单位编制的施工组织设计中的安全技术措施、施工现场临时用电方案、及专项施工方案。其主要内容包括:
? 编审程序的审查:编制、审核及批准签署齐全有效;
? 编制内容的审查:与安全生产有关内容应符合工程建设强制性标准。
? 应组织专家论证的专项施工方案，必须有专家书面论证审查报告，论证审查报告的签署齐全有效;
? 审查施工现场及毗邻建筑物、构筑物和地下管线等的专项保护措施:
? 查阅建设单位向施工单位提供的施工现场及毗邻区域内地上、地下管线资料和相邻建筑物、构筑物、地下工程有关资料的移交单及附件，并保留相关资料;
? 开工前，监理工程师应审查施工单位制定的对毗邻建筑物、构筑物和地下管线等专项保护措施，经总监理工程师签认后实施。
? 审核施工单位特种作业人员的操作资格证书。
? 检查施工单位是否针对施工现场实际情况制定应急救援预案，是否建立应急救援体系。
? 对施工单位拟用的起重机械的性能检测报告、验收许可及备案证书、安装单位企业资质及安装方案进行程序性核查;经项目监理部对其验收程序进行核查，签认后施工单位方可投入使用;拆卸前项目监理部应对施工单位所报送的资料(包括拆卸方案和拆卸单位的企业资质等)进行程序性核查，签认后施工单位方可进行拆卸。
? 检查其他施工机械设备的进场安装验收手续，并在相应的报审表上签署意见。
? 审核施工单位安全防护、文明施工措施费用的使用计划。
? 督促施工单位做好施工前现场人员的安全生产教育及各阶段的安全技术交底工作。
? 巡视检查施工现场的安全生产设施的搭设情况，对施工单位安全生产设施的验收手续进行核查。
? 审查施工单位对危险源的识别、评价及对重大危险源的确定，总监理工程师应在危险性较大分部分项工程确认报审表上签认。
? 核查施工单位安全生产准备工作，由总监理工程师在《工程开工安全工作报审表》上签署意见。
5、检查验收制度
根据现行法律法规规范要求，项目监理部建立了检查验收制度。
? 每天巡视检查施工过程施工单位安全保证体系落实情况;主要是项目经理、安全管理人员在岗情况，重点安全风险控制部位安全管理人员到位检查情况;检查施工现场各种安全标志和的安全防护设施是否符合工程建设强制性标准要求。
? 抽查施工单位所进行的“三级安全教育情况”和“班前安全活动”情况，施工单位每月应将接受“三级安全教育人员”花名册上报项目监理部备案。施工单位的花名册中应能反映员工的健康情况，并妥善保管员工的健康证明。没有经过安全教育的员工，
严禁上岗作业。
? 监督施工单位在施工前对所有参加施工的作业人员的安全技术交底，要求将交底、签字记录上报项目监理部备案。
? 抽查现场特种作业人员的特种作业操作资格证书，无合法有效的特种作业操作资格证书者不得上岗。
? 施工单位应建立进场设备台账，加强对进场设备的管理，并做好验收记录，及时更新，保证设备运行安全。监理不定期抽查进场设备验收记录，无设备验收记录严禁使用。对于特种设备，无合法有效的许可证件不得使用。
? 易燃、易爆、危险物品等应妥善保存，无合法有效的手续的，不得进入施工现场;已进入现场的，应立即移出，不得使用。
? 对危险性较大的分部分项工程的全部作业面，应巡视到位。
? 当施工单位对已批准的安全技术措施及专项施工方案进行调整、补充或变动时，要求施工单位按原程序重新报审。
? 审核施工总承包单位报送的分包单位安全生产条件报审表，并签署意见。经项目监理部审核后，分包单位方可进场。
6、事故处理报告制度
工程现场发生质量安全事故，项目监理部应按《生产安全事故报告和调查处理条件》，在规定的时间内，向有关方报告，并参加或配合事故调查、分析、处理，严禁隐瞒事故。
?、发生人身死亡和重伤事故、重大及以上设备事故或发生施工原因导致一般及以上事故、或发生负主要责任的重大交通事故、或发生重大火灾事故，应要求事故(肇事)单位立即报告项目监理部，监理部应立即用电话、传真或电子邮件向建设单位和监理单位本部报告，在1小时内将情况进一步核实后进行补充报告，并协助工程建设单位在16小时内提交书面事故报告。
?、发生一般事故，应要求事故单位24小时内向项目监理部报告，并在3天内提交书面事故报告，项目监理部在接到事故报告1周内，将事故报告及审核意见送交建设单位。
项目监理部应监督施工单位按“四不放过”原则进行安全事故处理，并按规定进行工程安全事故统计，保留事故报告、处理、统计的所有记录。
?、发现施工承包商的安全文明施工管理混乱，或拒不接受监理部的管理，应按有关规定进行罚款或向工程建设部建议在听拨付工程款甚至中止工程承包合同;对不服从管理或严重违章作业、野蛮施工、管理混乱的分包商，应要求施工承包商立即中止合同，限期清退并严禁再次进场施工。
?、质量、安全事故分类和等级划分应参照国家有关规定。
7、信息资料归档管理制度
安全监理信息管理是建设项目信息管理的重要组成部分，按照监理规划要求施行统一管理。
? 项目监理部使用统一的安全监理表示按《河北省工程建设标准 建筑施工安全技术资料管理标准》(DB13J/101—2009)，规范信息管理工作。
? 安全监理资料应纳入本项目监理部的过程监理资料管理。
? 制订的安全监理文件适用法律和标准要具体，并做必要的论证。对适用的法律依据，要载明法律依据的全称、发文号和发文时间(或生效时间)，引用条文要具体到条款项。对适用的标准依据，要在名称标准依据的全称、标准号和生效时间，引用条文要具体到章节条款项。
? 项目监理部应注重使用影响资料记录施工现场安全生产重要情况和施工安全事故隐患。
? 收发文按下列程序:
